Sumeet Kad
Reduced costs, improved healthcare outcomes
estersarquella
Improved health and welfare results
estersarquella
Improved user experiences
estersarquella
Greater efficiency
Sarah Jones
better patient care that is tailored to the individual, with better patient outcomes. Reduced patient stay in hospitals, providing tailored care in the home.
Brenda Decker
Access to better outcomes for care
estersarquella
Improvements in partnership working
Sumeet Kad
How does this lead to economic development?
Paul Dommel
Ability for fragile elderly to live longer, safely, independently.
Rizwan Khaliq
all with the goal of improved citizen services and engagement
Rich Varos
Improving outcomes for the citizen/patient, creating better quality of life
Paul Dommel
Outside of healthcare, the ability to work with people who may be mentally ill, in and out of homelessness or prison, to coordinate services required to get them re-engaged and working.
Paul Dommel
I was in India last week. As an extreme example, I learned that people with disabilities work 78% in the informal economy.
Curtis Clark
@PaulDommel Critical focus areas...
Sumeet Kad
@PaulDommel Great point. Mental ill patients special care and technology can provide that.
Paul Dommel
So, lots of room for increased wages and better opportunities for them and expanded tax base for government
Hector Upegui
allows to put the person in the center, giving back the person the right of choice
Heather Fraser
@sumeetkad Great presentation at NHS Confederation Conference this year by Alison Cameron on integrated care and mental illnes http://www.nhsconfed...
Hector Upegui
a better coordination in terms of health and social helps also to improve society cohesiveness
John Crawford
Less citizen dependency on medical care, an enhanced relationship with the care team, improved job satisfaction

Q1. What is integrated care and why are we hearing so much about it? http://www.via-cc.at...
Paul Dommel
Aging populations and huge increases in chronic diseases consume ever greater share of government budgets.
Paul Dommel
According to the WHO, by 2050 four times as many people over the age of 60 will need help caring for themselves than is the case today.
Brenda Decker
While I agree with aging populations, I also believe the consumer is demanding it rather than starting over with every care provider.
Paul Dommel
Right and consumer interest will play a big role in adoption.
Curtis Clark
I think another significant value of integrated care will be to focus on younger populations and focus on healthy lifestyles, to reduce long term costs
Sumeet Kad
How emerging economies can be helped through this?
estersarquella
increasing population with complex care needs requires new models of care
John Crawford
I agree with Brenda, it is about information sharing and teamwork, and we can all benefit - but we absolutely need it now for complex citizens
estersarquella
@Curtis_Clark_28 Do agree! Is it Integrated Care only about health and social care?
Angus McCann
Integ Care ensures that patients don't fall between the cracks in fragmented systems
Paul Dommel
Aging, homelessness, substance abuse, physical disability, and chronic disease all add to costs of care.
Dr Vikram V
Good point, its a question of being effective with care outcomes
David Zaharchuk
The age-dependency ratio is expected to rise an additional 10% through 2050 – placing even greater pressures on the budgets and resources of governments
Paul Dommel
@estersarquella Definitely not, but it is easiest place for some to understand. Employment, housing, mental health, are all related as well.
Sumeet Kad
@estersarquella No..I believe it involves the entire healthcare ecosystem and various stakeholders
Curtis Clark
@estersarquella I think a broad focus on both health and social issues would drive important value and impact long term costs.
Rich Varos
Agreed - - need to collaborate across traditional areas - - gaining the hidden insights with a more complete set of information
estersarquella
@sumeetkad @PaulDommel Yeah! Most of the key elements are shared when we consider complex care as common framework (employment, housing, social care, healthcare)
Rizwan Khaliq
all of the focus on operational effectiveness and improve services will impact economic vitality of a region/city
Hector Upegui
it is also about what we understand by health and by social.
Angus McCann
@estersarquella No... needs to bring together multiple agencies depending on how structured in a country. eg Housing. Even the Fire Service in some settings now doing home safety checks
Paul Dommel
Thats right. Fragmentation is a big issue. Lots of well-intentioned programs. They are hard for policy makers to fully understand and nearly impossible for individuals and their families.
Rakesh Ravindran
combining data integration, analytics and coordination of care capabilities into a single ready-to-use offering is what we need
Rizwan Khaliq
it will lead to attracting innovators and net plus revenue streams for governments, allowing for continued improved and expanding services
John Crawford
The work of Sir Michael Marmot, Harry Burns and other public health experts shows that, for many people, their environment, education and social connections lead to poverty and poor health, but are these problems that better integrated care can tackle?
Angus McCann
And not just statutory services, voluntary organisations have huge part to play
Brenda Decker
I agree - the voluntary groups have a huge piece of this puzzle are sometimes the last to participate.
Angus McCann
@PaulDommel Another perspective on integrated care is that the patient / citizen should not have to repeat their story over and over to different care professionals
Rakesh Ravindran
improve outcomes while controlling cost
estersarquella
“I can plan my care with people who work together to understand me and my carer(s), allow me control, and bring together services to achieve the outcomes important to
me.” by @NVTweeting https://www.england....
Rakesh Ravindran
See how integrated care is improving social and economic vitality in Spain http://bit.ly/1RmG7i...
John Crawford
Having good child health, education and employment prospects provides the best possible foundation for good health, without these elements we can expect to see more people failing to thrive
Angus McCann
'Foundation' is a key word there, countries need to take a long term view and enable this through legislation, funding etc
estersarquella
@raks777 By now i'd say it can contribute to better use of resources in Spain more than economic vitality even it may affect the superusers market in a medium-term #ownview
Hector Upegui
I see some confusion in different territories on what integrated care and coordinated care is... any thoughtss?
